How to Apply

If you are confident that your organization meets the program eligibility criteria and are interested in a mobile adoption van, you are encouraged to submit a proposal to the Foundation.

To submit an initial proposal, please fill out our Eligibility Assurance Form found on our Resources page .

The timeline for mobile adoption van funding is as follows:

  1. Complete and submit eligibility assurance form (see Resources) by emailing
  2. Mobile Adoption Van Coordinator will schedule a call to discuss eligibility assurance form and general requirements.
  3. Send requested supplemental documents as an attachment (see Resources).
  4. After successful review of requested supplemental documents, obtain preliminary commitment by Foundation.
  5. Receive approval by appropriate town / fiscal authority and provide to Mobile Adoption Van Coordinator.
  6. Receive funds from Foundation.
  7. Place order for van!
